Kimbery Chu, LCSW, DCSW is an out-of-network provider, not on any insurance panels. Typically, her services are insurance reimbursable. It is highly recommended that patients be aware of their current mental health insurance benefits, including coverage after deductibles are met. Specific plan information is available on most insurance company’s websites and also can be obtained by calling one’s insurance company directly. If you intend to submit invoices for insurance reimbursement, please advise ahead of time so that insurance coded bills can be provided for you.
